What sort of information can I request?
You can request any information that the CCG holds, that does not fall under an exemption.
If the information is your own personal data, then you should make a subject access request under the Data Protection Act (DPA) 1998, and not under the FOI Act.
How do I make a request for information?
Freedom of Information (FOI) requests are managed by South, Central and West (SCW) Commissioning Support Unit.
Requests should be made in writing and directed to email@example.com.
What information must I include in my request?
The FOI Act requires certain information to be supplied before the CCG can respond to your request:
- Your real name – we do not have to respond to requests submitted under a pseudonym
- Your address (email addresses are acceptable)
- A description of the information you wish to obtain
Can my request be refused?
Our commitment to publish information excludes any information which can legitimately be withheld under the exemptions set out in the NHS Openness Code or the Freedom of Information Act e.g. the protection of commercial interests and personal information under the Data Protection Act 1998. This applies to all classes within the publication scheme.
What information do we publish?
Our publication scheme sets out the CCG’s commitment to make the following classes of information routinely available:
- Who we are and what we do,
- What we spend and how we spend it,
- What are priorities are and how we are doing,
- How we make decisions,
- Our policies and procedures,
- Lists and registers, and
- The services we offer.
Do I need to pay costs?
The cost of accessing most information will be free, but there may be a charge for some information. The charges will vary according to how the information is made available and the format. We will let you know the cost and charges that will have to be paid in advance. We will not provide printouts of other organisation’s websites.
